The group's administrative processes were paper-first in a way that created real compliance exposure rather than just inefficiency. Provider credentialing involves collecting licences, certifications and insurance documents with expiry dates, and the tracking mechanism was a spreadsheet that someone remembered to review. An expired credential that goes unnoticed is a regulatory and insurance problem, not an admin one. Incident reports were completed on paper, scanned and emailed, which meant no reliable aggregate view of what was going wrong across sites and no way to demonstrate a closed loop on follow-up actions. Everything containing patient information fell under HIPAA, so any solution had to keep protected health information inside their existing compliance boundary — a significant argument against introducing another vendor. Budget was the other constraint: they had no appetite for a custom development programme, and the CFO's position was that they were already paying for Microsoft 365 and should use it. The clinical staff were also, reasonably, sceptical of anything that added clicks to their day.

Automated workflows inside the existing M365 boundary
We built the workflows entirely within the group's existing Microsoft 365 tenant, so protected health information never left a boundary already covered by their business associate agreement with Microsoft — that scoping decision was the reason the project was approved at all. Credentialing runs on a SharePoint list with document libraries per provider, and Power Automate flows watch expiry dates and escalate on a schedule: notice to the provider, then to their manager, then to compliance, with the chain recorded. Nothing depends on someone remembering to check. Incident reporting is a Power App usable on a phone at the point of the incident rather than a form completed later from memory, with routing to the right reviewer by incident type and site, and an explicit closure step so the follow-up loop is demonstrable. Equipment checks work the same way, with scheduled prompts and photo capture. Access is scoped by site and role through existing Microsoft 365 groups, so joiners and leavers are handled through the identity processes the group already runs rather than a separate user list.
